Remote patient monitoring is a way for your doctor to monitor certain vital signs and disease risk factors remotely, while you’re in the comfort of your own home. Doing so means you can attend in-person visits less often and save time and money.

The clinical team identifies patients with certain chronic conditions that may benefit from our remote patient monitoring program. Your healthcare provider will use remote patient monitoring to collect a wide range of patient health data to make sure that our most vulnerable patients stay out of the hospital or nursing homes. This includes blood pressure, heart rate, vital signs, weight, and blood sugar levels.

You will keep a health-monitoring device at home. This could be as simple as a blood pressure monitor, or it could be a “wearable” that constantly tracks vital signs, such as respiration rate, pulse, body temperature, and step counts.

Your doctor will help make sure you know how to properly use the device so that you’re getting the most accurate results. Then, the collected data gets sent to your doctor. They can get alerts if your device indicates trouble, such as an increase in temperature (i.e., you have a fever) or drops in respiration levels.

Remote patient monitoring allows your provider to monitor health parameters more often to help detect health problems early, before serious complications arise. For example, your doctor can track your blood pressure to make decisions about changing your medication dosage or adding or discontinuing a certain medicine. 

Using these monitoring devices at home instead of coming to the office is much more convenient for you, and still gives you facilitated access to medical care.

We check your blood pressure, other vital signs, and body weight to get a baseline reading prior to home patient monitoring.

We may also complete a physical examination, as well as review your medical history, medication regimen, and symptoms, if you have any. Our specialist might recommend you undergo urine tests, blood tests, or other diagnostic procedures. 

The patient is provided with a device, trained on the use of the device and receives regular calls from our caring clinical team. The most common RPM devices are blood pressure monitors, weight scales and blood glucose meters,  and pulse oximeters.
